Congenital stenosing tenosynovitis
Is an abnormal appearance of the thumb mostly in flexed or semi flex position.
Inability to extend the thumb fully ,A nodule maybe palpated the at the base of the thumb/finger.
The disproportion in the tunnel of the A1 Pulley and the flexor fibrous sheath
Obstructing the tendon movement. May be during the grasp reflex period keep the thumb /finger in flexion position for a long period of time, thus limits the movement of the thumb/fingers.
Parents recognized the abnormality at the first year of age or later. One third of the cases can be treated by stretching, passive movements massage or splinting. The rest need operation to cure and correct the deformities. Surgery is the only curative treatment and the results are very good.
Percutaneous release of congenital trigger thumbs/fingers patients under local anesthesia of 29 children of 38 fingers , 16 boys and 13 girls. 28 Thumbs , 7 middle fingers and 3 ring fingers. No case of indexes and little finger. The results are very good as shown in the photos.The results are good even being released at older ages.(12, 14, 25 and 44 year of age).anyway the deformities should be corrected by age of three before pre kindergartens to prevent manual skill developing in pre school period.
